Summary
MRI texture analysis can detect hippocampal abnormalities in patients with hippocampal sclerosis (HS), a common feature of mesial temporal lobe epilepsy (MTLE). While texture analysis showed differences between affected and healthy hippocampi, it did not distinguish between affected and contralateral hippocampi.
Area of Science:
- Neurology
- Radiology
- Medical Imaging
Background:
- Mesial temporal lobe epilepsy (MTLE) often presents with difficult-to-treat seizures.
- Hippocampal sclerosis (HS) is a common pathological finding in MTLE.
- Quantitative MRI techniques show promise for detecting and measuring HS.
Purpose of the Study:
- To assess if MRI texture analysis can identify hippocampal abnormalities in patients with pathologically confirmed HS.
- To evaluate the utility of texture analysis in the context of MTLE and HS.
Main Methods:
- Texture analysis was performed on preoperative 3-mm coronal T1-IR MRIs of the hippocampal head in 19 patients with surgically treated MTLE and histopathologically diagnosed HS.
- The software MAZDA was used for texture analysis.
- Data were compared to 78 normal hippocampi from 39 healthy volunteers using multivariate analysis of variance.
Main Results:
- Texture analysis parameters largely differentiated hippocampi with HS and contralateral hippocampi from normal hippocampi.
- However, post hoc comparisons revealed no significant differences between the HS hippocampi and their contralateral counterparts.
- This suggests widespread changes beyond the primarily affected side.
Conclusions:
- MRI texture analysis provides evidence of altered texture in hippocampi affected by HS.
- The findings support the hypothesis that hippocampal damage in MTLE may be bilateral.
- Further research is required to determine the effectiveness of texture analysis in lateralizing hippocampal damage in MTLE.
