Summary
Resection of pericardial diverticulum was performed in two women. Diagnostic procedures involving fluoroscopy and indocyanine green (ICG) instillation aided in differentiating the lesion from pericardial cysts.
Area of Science:
- Cardiology
- Thoracic Surgery
- Diagnostic Imaging
Background:
- Pericardial diverticulum is a rare congenital anomaly.
- Accurate diagnosis is crucial for differentiating it from other mediastinal masses, such as pericardial cysts.
Observation:
- Two cases of surgical resection of pericardial diverticulum are presented.
- Case 1: A 66-year-old woman with a lesion in the right upper mediastinum.
- Case 2: A 20-year-old woman with a lesion at the right anterior cardio-phrenic angle.
Findings:
- Fluoroscopic observation showed the lesion's shape varied with patient positioning in Case 1.
- Indocyanine green (ICG) instilled into the lesion diffused into the pericardial cavity in Case 2.
- These diagnostic methods proved useful in distinguishing pericardial diverticulum from pericardial cyst.
Implications:
- Surgical resection is a viable treatment option for symptomatic pericardial diverticula.
- Dynamic fluoroscopy and ICG instillation are valuable adjuncts for preoperative diagnosis.
- Improved diagnostic accuracy can prevent unnecessary interventions for misdiagnosed conditions.
