Summary

Donepezil significantly improved cognition in vascular dementia (VaD) patients. This acetylcholinesterase inhibitor was well-tolerated, offering a potential new treatment for VaD.

Background:

  • Vascular dementia (VaD) is the second most common dementia type, often resulting from stroke or small vessel disease.
  • Donepezil, an acetylcholinesterase inhibitor, is approved for Alzheimer's disease (AD) but its efficacy in VaD is under investigation.

Purpose of the Study:

  • To evaluate the efficacy and tolerability of donepezil in patients with vascular dementia.

Main Methods:

  • Two randomized, double-blind, phase III trials were conducted.
  • Participants received either donepezil (5mg or 10mg once daily) or a placebo for 24 weeks.
  • Cognitive function was assessed using the Alzheimer's Disease Assessment Scale-cognitive subscale (ADAS-cog). Global functioning was measured by the Clinician's Interview-Based Impression of Change-plus version score.

Main Results:

  • Donepezil (5mg and 10mg) significantly improved cognitive function compared to placebo, with effect sizes of -1.79 and -2.28 on the ADAS-cog, respectively (p < 0.001).
  • Significant improvement in global functioning was observed with 5mg donepezil (p = 0.003), but not with 10mg.
  • Donepezil was generally well-tolerated, with mild to moderate adverse events like diarrhea and nausea being most common.

Conclusions:

  • Donepezil demonstrates significant cognitive benefits in patients with vascular dementia.
  • The 5mg dose also showed a positive impact on global functioning.
  • Donepezil is a well-tolerated treatment option for vascular dementia.
