Background:

Observation:

  • A 36-year-old woman with known complete situs inversus presented with acute abdominal symptoms initially suggestive of appendicitis.
  • Abdominal CT indicated diverticulitis; however, subsequent colonoscopy revealed a sigmoid colon tumor.
  • A left colectomy was successfully performed via laparotomy, with pathological examination confirming T3N0 colonic adenocarcinoma.

Findings:

  • The patient's complete situs inversus, diagnosed in childhood, did not impede surgical intervention for colonic adenocarcinoma.
  • Diagnostic difficulties in acute abdominal presentations are noted when situs inversus is undiagnosed in adults.
  • The association between situs inversus and colonic tumors is considered fortuitous in current literature.

Implications:

  • Surgical adaptation to situs inversus is generally manageable, even in complex cases.
  • Undiagnosed situs inversus can lead to significant diagnostic delays and misinterpretations in emergency medicine.
  • This case underscores the importance of considering rare congenital anomalies in differential diagnoses for abdominal pathologies.
