Current Posttraumatic Stress Disorder (PTSD) diagnostic criteria may exclude many individuals. Research suggests recognizing partial PTSD and subthreshold PTSD syndromes could improve care for trauma survivors.

Area of Science:

  • Psychiatry
  • Clinical Psychology
  • Trauma Studies

Background:

  • The current diagnostic criteria for Posttraumatic Stress Disorder (PTSD) may be too restrictive.
  • This can lead to underdiagnosis and lack of access to care or compensation for trauma victims.
  • The concepts of Subthreshold PTSD (PPTSD) and partial syndromes have been proposed to address this gap.

Purpose of the Study:

  • To investigate the prevalence of subthreshold and partial PTSD syndromes in trauma-exposed populations.
  • To assess symptom patterns within different diagnostic criteria (B, C, and D) of PTSD.
  • To determine if these syndromes warrant specific nosological categories.

Main Methods:

  • Utilized the Composite International Diagnostic Interview (CIDI) PTSD-module for data collection.

Related Experiment Videos

  • Included 130 fire victims and 55 car accident victims exposed to trauma.
  • Employed hierarchical class analyses to assess syndrome patterns and symptom clusters for criteria B, C, and D.
  • Main Results:

    • PTSD diagnosis was met by 18.4% to 22.7% of subjects, depending on the threshold for criterion C.
    • Subthreshold syndromes were identified in 8.7% of subjects.
    • Partial syndromes were found in 60.7% of subjects, with 16.7% experiencing clinically significant impairment.

    Conclusions:

    • A significant number of individuals present with partial and subthreshold PTSD syndromes.
    • These syndromes, particularly those with clinically significant impairment, should be considered distinct nosological categories or specified PTSD subcategories.
    • Recognizing these categories can improve identification and support for a broader range of trauma survivors.
