Pituitary apoplexy, a rare complication of pituitary adenomas, presents with sudden headache and vision loss. Prompt diagnosis and treatment, including surgery, improve visual outcomes but not hormonal deficiencies.

Area of Science:

  • Neurology
  • Endocrinology
  • Neurosurgery

Background:

  • Pituitary apoplexy is a rare but serious complication of pituitary adenomas.
  • It results from hemorrhage or infarction within the pituitary tumor.
  • Typical symptoms include sudden severe headache, visual impairment, and altered consciousness.

Observation:

  • Eight patients with pituitary apoplexy were treated over ten years.
  • Clinical diagnosis was confirmed in all cases.
  • Common symptoms were severe headache (7/8), vomiting (7/8), and visual disturbances (8/8).
  • In six patients, apoplexy was the initial presentation of a pituitary adenoma.
  • Neuroimaging (MRI/CT) confirmed pituitary stroke in seven patients.

Findings:

  • Seven patients underwent trans-sphenoidal surgery, with only one requiring urgent intervention.

Related Experiment Videos

  • All patients showed significant improvement in visual function post-surgery.
  • No improvement in preoperative endocrine disturbances was observed in any patient.
  • Implications:

    • Early diagnosis and prompt hormonal therapy are crucial for managing pituitary apoplexy.
    • Trans-sphenoidal surgery, whether urgent or delayed, is a key treatment component.
    • While surgery improves visual deficits, endocrine dysfunction requires ongoing management.
