Epilepsy surgery can lead to new-onset obsessive-compulsive disorder (OCD) in some patients, particularly those with mesial temporal lobe epilepsy (MTLE) and pre-existing obsessive traits. Careful monitoring post-surgery is recommended.

Area of Science:

  • Neurology
  • Psychiatry
  • Epileptology

Background:

  • Epilepsy surgery (ES) can precipitate or worsen psychopathology.
  • While OCD recovery post-ES is documented, new-onset OCD after ES has not been reported, especially in mesial temporal lobe epilepsy (MTLE).

Purpose of the Study:

  • To report the first cases of de novo obsessive-compulsive disorder (OCD) following ES in patients with MTLE.
  • To highlight the potential risk of developing OCD post-ES in specific patient groups.

Main Methods:

  • Retrospective case series.
  • Identified five MTLE patients with pre-operative obsessive personality traits.
  • Assessed for OCD development within two months post-ES.

Main Results:

Related Experiment Videos

  • Two out of five MTLE patients developed OCD post-ES.
  • No significant differences in epilepsy characteristics were found between patients who developed OCD and those who did not.
  • All patients achieved seizure freedom, but OCD patients reported a diminished quality of life.

Conclusions:

  • Patients with MTLE and pre-existing obsessive traits require careful post-ES monitoring for potential OCD development.
  • De novo OCD can significantly impact quality of life after successful epilepsy surgery.
