Summary

Deep lupus erythematosus, a rare condition typically affecting middle-aged women, was observed in an 11-year-old girl. Hydroxychloroquine effectively treated her subcutaneous nodules and ulcerations, indicating its therapeutic potential in pediatric cases.

Area of Science:

  • Dermatology
  • Rheumatology
  • Immunology

Background:

  • Deep cutaneous lupus erythematosus (DLE), also known as lupus erythematosus profundus or panniculitis, is uncommon.
  • It typically presents in middle-aged women, making pediatric cases exceptionally rare.

Observation:

  • A unique case of DLE in an 11-year-old girl featured facial subcutaneous nodules and ulcerations.
  • Histopathology revealed lobular lymphocytic panniculitis with hyalinization and perivascular infiltrates.
  • Direct immunofluorescence showed granular IgM deposits at the dermo-epidermal junction.

Findings:

  • Laboratory results included leukopenia, neutropenia, and positive antinuclear and anti-native DNA antibodies.
  • Renal function and complement levels were normal, excluding other causes of panniculitis.

Related Experiment Videos

  • The patient's lesions significantly improved within 3 weeks of hydroxychloroquine treatment and did not relapse after withdrawal.
  • Implications:

    • This case highlights DLE's potential presentation in pediatric patients, particularly young girls.
    • Hydroxychloroquine demonstrated efficacy in treating DLE in a pediatric patient.
    • Further analysis of 17 pediatric DLE cases identified characteristic facial and shoulder lesions, often resulting in atrophic scarring.
