Summary

Piperacillin showed good intraocular diffusion into the aqueous humor, particularly in infected eyes of rabbits. This suggests potential efficacy for ocular infections when used early and in combination therapy.

Area of Science:

  • Ophthalmology
  • Pharmacology
  • Microbiology

Context:

  • Cataract surgery and vitrectomy patients (n=42) were studied.
  • Piperacillin was administered intravenously at 4 g/injection.
  • Both single and double doses (12 hours apart) were evaluated.

Purpose:

  • To investigate the intraocular diffusion of piperacillin.
  • To assess piperacillin levels in aqueous and vitreous humor.
  • To evaluate piperacillin's penetration in infected versus non-infected ocular tissues in rabbits.

Summary:

  • In human patients, peak piperacillin levels in aqueous humor were 7.3 mg/l one hour post-injection, while vitreous levels were low (<0.6 mg/l).
  • In infected rabbit eyes, increased piperacillin diffusion and decreased elimination were observed in the aqueous, iris, and cornea.
  • Albino rabbits showed less conclusive results regarding intraocular diffusion differences between infected and healthy eyes.

Impact:

  • Piperacillin demonstrates good diffusion into the aqueous humor, especially in infected eyes.
  • Findings suggest piperacillin's potential utility in treating ocular infections.
  • Early initiation and combination therapy with other antimicrobials are recommended for optimal outcomes.
