Summary

This study reveals that Hy- blood cells can express the Joa antigen weakly, challenging previous assumptions about blood group typing. This finding improves understanding of rare blood group antigen expression.

Background:

  • The identification of antibodies like anti-Hy and anti-Joa is challenging due to poorly characterized reagent red blood cells (RBCs).
  • Historically, RBCs with the Hy- phenotype were typed as Gy(a+w), Hy-, Jo(a-), and those with the Jo(a-) phenotype as Gy(a+), Hy+w, Jo(a-).

Observation:

  • A case involving a 71-year-old woman identified an anti-Joa antibody in her serum.
  • This antibody reacted weakly with Hy- RBCs, contradicting established typing.
  • Adsorption and elution studies confirmed the presence of a weak Joa antigen on Hy- RBCs.

Findings:

  • The patient's RBCs were serologically determined as Gy(a+), Hy+, Jo(a-).
  • DNA analysis confirmed the patient was homozygous for the JO allele, consistent with Jo(a-) status.
