Area of Science:

  • Gastroenterology
  • Oncology
  • Diagnostic Cytopathology

Background:

  • Accurate tissue diagnosis is critical for managing patients with malignant biliary obstruction.
  • Endoscopic retrograde cholangio-pancreatography (ERCP) is a common procedure for biliary interventions.

Purpose of the Study:

  • To prospectively evaluate the utility of wire-guided brush cytology in diagnosing malignant biliary obstruction.

Main Methods:

  • 30 patients with malignant biliary obstruction underwent wire-guided brush cytology during ERCP.
  • Samples were collected from the stricturous segment of the bile duct.
  • Cytological analysis was performed to detect malignancy.

Main Results:

  • Brush cytology was positive for malignancy in 26.7% of cases and suspicious in 10.3%.

Related Experiment Videos

  • Sensitivity varied by cancer type: 25% for gallbladder, 50% for pancreatic, and 50% for cholangiocarcinoma.
  • Overall sensitivity for diagnosing malignant biliary strictures was 36.7%.
  • Conclusions:

    • Wire-guided brush cytology has a limited sensitivity in diagnosing malignant biliary strictures.
    • Further larger-scale studies are necessary to validate these findings and improve diagnostic accuracy.
