Refractory anemia with excess blasts in transformation (RAEB-t) is a rare hematologic malignancy.
RAEB-t in young adults presents unique diagnostic challenges.

Megaloblastic erythropoiesis can be a presenting feature of RAEB-t.
Two cases of RAEB-t with megaloblastic erythropoiesis in young adults were observed.

Findings:

RAEB-t with megaloblastic features necessitates careful differential diagnosis.
Distinguishing RAEB-t from vitamin B12/folic acid deficiency and acute erythroleukemia is crucial.
