Summary
Diabetic patients, especially the elderly, may develop hepatic abscesses. Early detection through systematic screening is crucial for effective treatment and management of diabetes mellitus complications.
Area of Science:
- Hepatology
- Infectious Diseases
- Endocrinology
Background:
- Hepatic abscess is an increasingly recognized complication in diabetic patients.
- Diabetes mellitus, particularly in the elderly, presents a higher risk for this septic localization.
Observation:
- Two elderly diabetic patients with type 2 diabetes mellitus decompensation presented with sub-febrile state and infectious syndrome, but lacked typical hepatic signs.
- Systematic hepatic sonography revealed abscesses in both patients.
- Klebsiella oxytoca was identified in one patient via liver puncture.
Findings:
- Both patients showed positive outcomes with antibiotic treatment.
- One patient also required surgical draining, indicating varied treatment approaches.
- The origin of the abscesses was attributed to biliary and arterial sources in the respective patients.
Implications:
- These cases highlight the importance of considering hepatic abscess in diabetic patients with unexplained infectious syndromes.
- Systematic evaluation for hepatic abscess is recommended in diabetic patients presenting with infectious symptoms.
- Prompt diagnosis and appropriate management, including antibiotics and potentially surgical intervention, are key for favorable outcomes.
