Summary

Routine bladder biopsies in men with bladder outlet obstruction show correlation with impaired bladder capacity and compliance. However, their routine evaluation is not recommended for diagnosis.

Area of Science:

  • Urology
  • Pathology
  • Medical Diagnostics

Background:

  • Bladder outlet obstruction (BOO) presents with lower urinary tract symptoms.
  • Histopathological changes in bladder biopsies may offer insights into BOO severity.
  • Urodynamic parameters are crucial for assessing BOO and bladder function.

Purpose of the Study:

  • To determine if microscopic bladder biopsy findings correlate with urodynamic parameters in men with BOO.
  • To assess the reliability of two pathologists in interpreting bladder biopsies.
  • To evaluate the clinical utility of routine bladder biopsies in BOO evaluation.

Main Methods:

  • Sixty-three male patients with suspected BOO underwent bladder biopsies.
  • Two pathologists independently evaluated biopsies for fibrosis, elastosis, muscle degeneration, and inflammation.

Related Experiment Videos

  • Histologic diagnoses were correlated with clinical and urodynamic data.
  • Main Results:

    • Abnormal bladder biopsy findings correlated with reduced bladder compliance and capacity.
    • Histologic abnormalities did not correlate with Schäfer class, urethral resistance, or prostate volume.
    • Reasonable intraobserver and interobserver reproducibility was noted (kappa 0.35 and 0.50).

    Conclusions:

    • While abnormal bladder biopsies indicate impaired bladder function, routine use is not justified in BOO evaluation.
    • The correlation is significant but not sufficient for routine diagnostic protocols.
    • Further research may explore specific histological markers for targeted BOO management.
