Summary

Topical nasal steroids, like fluticasone propionate and beclomethasone dipropionate, did not increase infection rates after endoscopic polypectomy. Steroid use also showed lower rates of nasal polyp recurrence compared to saline lavage alone.

Background:

  • Topical nasal steroids are common treatments for rhinitis and polyposis.
  • An increased incidence of infection was noted with fluticasone propionate post-endoscopic polypectomy.

Purpose of the Study:

  • To evaluate the prevalence of nasal and paranasal infections with topical nasal steroid use after endoscopic polypectomy.
  • To compare the recurrence rates of nasal polyposis among different treatment groups.

Main Methods:

  • A prospective, comparative, open-label, longitudinal study was conducted.
  • 162 patients undergoing endoscopic polypectomy were randomized into three groups: saline lavage only, fluticasone propionate (400 mcg/day), or beclomethasone dipropionate (600 mcg/day).
  • Infection prevalence and polyp recurrence were monitored over a minimum of 12 months.

  • Only three patients developed infections within the first three months (2 placebo, 1 beclomethasone).
  • Nasal polyp recurrence rates were 44% (saline), 15% (fluticasone), and 26% (beclomethasone).
  • Fluticasone propionate demonstrated the lowest recurrence rate.

Conclusions:

  • Nasal steroid use does not appear to elevate infection risk following endoscopic polypectomy.
  • Topical nasal steroids may be beneficial in reducing nasal polyp recurrence after surgery.
