Summary

Severe hypothyroidism can cause acute renal failure and muscle breakdown (rhabdomyolysis). Prompt thyroid hormone replacement therapy can reverse these serious complications, highlighting hypothyroidism as a critical consideration in patients with unexplained renal and muscle issues.

Area of Science:

  • Endocrinology
  • Nephrology
  • Musculoskeletal Medicine

Background:

  • Hypothyroidism is frequently associated with muscular disorders, including hypothyroid myopathy and elevated muscle enzymes.
  • Acute renal failure (ARF) can present with diverse etiologies, necessitating a broad differential diagnosis.

Observation:

  • A 35-year-old male presented with myalgia and acute renal failure, characterized by a serum creatinine level of 2.4 mg/dl.
  • Further investigation revealed severe hypothyroidism, with a thyroid-stimulating hormone (TSH) level exceeding 500 uIU/ml, as the underlying cause of his symptoms.

Findings:

  • Thyroid replacement therapy led to the stabilization and subsequent improvement of both the patient's myalgia and serum creatinine levels.
  • This case underscores that hypothyroidism, though uncommon, is a recognized cause of rhabdomyolysis.

Implications:

  • Hypothyroidism should be considered in the differential diagnosis of patients presenting with acute renal failure, particularly when accompanied by elevated muscle enzymes.
  • Early recognition and treatment of hypothyroidism can prevent or reverse severe complications like rhabdomyolysis and acute kidney injury.
