Summary

Dobutamine can cause eosinophilia in patients with heart failure. Promptly stopping the drug can prevent unnecessary testing for hypereosinophilic syndrome.

Area of Science:

  • Cardiology
  • Hematology

Background:

  • Dilated cardiomyopathy and decompensated heart failure are serious conditions often requiring intensive medical management.
  • Eosinophilia, an elevated eosinophil count, can be associated with various conditions, including allergic reactions, infections, and hematologic disorders.

Observation:

  • A patient with dilated cardiomyopathy and heart failure presented with significant eosinophilia.
  • Extensive investigations for hypereosinophilic syndrome were initiated, including a myocardial biopsy.

Findings:

  • A second dobutamine infusion (rechallenge) confirmed dobutamine-induced eosinophilia.
  • This suggests dobutamine, a commonly used inotropic agent, can precipitate eosinophilia in vulnerable patients.

Implications:

  • Given dobutamine's widespread use in critically ill patients, recognizing drug-induced eosinophilia is crucial.
  • Discontinuation or substitution of dobutamine can avoid costly and invasive evaluations for hypereosinophilic syndrome.
