Symptomatic uncomplicated diverticular disease (SUDD) involves heightened rectal and sigmoid perception of distension, unlike asymptomatic diverticular disease (ADD). This increased visceral sensitivity is not linked to changes in colorectal wall compliance.

  • Pathogenesis of asymptomatic diverticular disease (ADD) and symptomatic uncomplicated diverticular disease (SUDD) remains unclear.
  • Investigating the roles of visceral perception and colorectal wall compliance in ADD and SUDD.

Purpose of the Study:

  • To determine if altered visceral perception or abnormal colorectal wall compliance contribute to ADD and SUDD.
  • To compare these factors between ADD patients, SUDD patients, and healthy controls.

Main Methods:

  • Utilized a dual barostat device to assess rectal and sigmoid sensations and compliance in 10 ADD patients, 11 SUDD patients, and 9 controls.
  • Measured perception and compliance curves via isobaric distensions before and after a liquid meal.
  • Monitored colonic response to eating by tracking barostat bag volumes.

Main Results:

  • SUDD patients exhibited increased rectal perception compared to controls and ADD patients.
  • Sigmoid colon perception was elevated in SUDD patients versus controls, but not significantly different from ADD patients.
  • Colorectal wall compliance and the colonic response to eating were similar across all groups.

Conclusions:

  • Symptomatic uncomplicated diverticular disease (SUDD), but not ADD, is associated with heightened visceral perception in both the sigmoid colon and the rectum.
  • This hyperperception in SUDD is independent of altered colorectal wall compliance.
