Summary

Diagnosing intraocular lymphoma can be challenging. Vitreous cytology combined with ancillary studies, like immunocytochemistry and gene rearrangement analysis, aids in detecting large B-cell lymphoma in the eye.

Area of Science:

  • Ophthalmology
  • Hematology
  • Cytopathology

Background:

  • Intraocular lymphoma, particularly large cell lymphoma involving the vitreous, presents diagnostic challenges.
  • Distinguishing between inflammatory infiltrates and neoplastic cells in vitreous samples is difficult without ancillary studies.

Observation:

  • A 75-year-old male presented with bilateral blurred vision.
  • Initial vitrectomy was inconclusive due to few neoplastic cells.
  • A subsequent vitrectomy with advanced analysis revealed large B-cell lymphoma.

Findings:

  • Immunocytochemical staining confirmed B-cell lineage.
  • Polymerase chain reaction demonstrated monoclonal B-cell populations.
  • These ancillary studies confirmed the diagnosis of large B-cell lymphoma.

Related Experiment Videos

Implications:

  • Vitreous cytology is a valuable tool for diagnosing intraocular lymphoma.
  • Ancillary studies significantly enhance the sensitivity of vitreous cytology.
  • Accurate diagnosis is crucial for timely and effective treatment of intraocular lymphoma.
