Background:

Lichen striatus is a rare, usually self-limiting skin condition.
Typically presents as unilateral papules and plaques in a linear distribution.

Observation:

A unique case of bilateral lichen striatus on the lower extremities is presented.
The condition demonstrated remarkable long-term persistence exceeding ten years.
Histopathological examination revealed a lichenoid tissue reaction with spongiosis and perivascular inflammation.

Findings:

Key histopathological findings included lymphocytic infiltrations around the eccrine duct.
The bilateral and chronic nature of this lichen striatus case is highly unusual.
Successful treatment was achieved with topical corticosteroid ointment.
