Summary

Surgical intervention effectively closes chronic macular holes, improving vision in most patients. Internal limiting membrane (ILM) peeling is crucial for successful single-operation macular hole closure.

Area of Science:

  • Ophthalmology
  • Retinal Surgery

Background:

  • Chronic macular holes, defined as full-thickness retinal defects in the macula lasting over a year, present significant challenges for visual recovery.
  • Surgical management of these long-standing macular holes is often considered less successful than for acute cases.

Purpose of the Study:

  • To evaluate the efficacy of pars plana vitrectomy in surgically treating chronic macular holes (duration > 1 year).
  • To determine the impact of internal limiting membrane (ILM) peeling on the success rate of macular hole closure.

Main Methods:

  • Retrospective review of 22 patients (23 eyes) who underwent pars plana vitrectomy for chronic macular holes.
  • Surgical techniques included gas tamponade (C3F8) with or without ILM peeling.
  • Preoperative and postoperative visual acuity, macular hole closure rates, and the role of ILM peeling were analyzed.

Related Experiment Videos

Main Results:

  • Macular hole closure was achieved in 83% of eyes (19/23) at a mean follow-up of 4.67 years.
  • Eyes undergoing ILM peeling demonstrated a significantly higher success rate for single-operation closure (81% vs. failure without peeling).
  • Visual acuity improved in 70% of patients, with 39% achieving 20/70 or better and 8.7% achieving 20/40 or better.

Conclusions:

  • Pars plana vitrectomy is an effective surgical option for closing chronic macular holes, leading to visual improvement in a majority of cases.
  • Internal limiting membrane (ILM) peeling is a critical factor associated with successful single-operation closure of chronic macular holes.
