Background:

  • Glomerular diseases are sometimes linked to thyroid disorders, with membranous glomerulopathy being the most common presentation.
  • Autoimmune thyroiditis can manifest with various systemic complications, including renal involvement.

Observation:

  • A 67-year-old man with primary hypothyroidism due to autoimmune thyroiditis developed nephrotic syndrome and acute kidney injury.
  • He presented with high antibody titers against thyroglobulin and microsomal thyroid antigen, and decreased C3/C4 complement levels.

Findings:

  • Renal biopsy revealed membranoproliferative glomerulonephritis with mesangial proliferation, a previously undescribed glomerular lesion associated with thyroid disease.
  • The patient showed dramatic renal function recovery, reduced proteinuria, and normalized complement levels after intravenous and oral steroid therapy.
