Primary central nervous system (CNS) lymphoma in AIDS patients is strongly linked to Epstein-Barr virus (EBV). This aggressive cancer has a poor prognosis and short survival rates.
Area of Science:
- Neuro-oncology
- Infectious diseases
- Immunology
Context:
- Primary central nervous system (CNS) lymphoma is the most frequent neoplasm in individuals with acquired immunodeficiency syndrome (AIDS).
- Understanding the characteristics and outcomes of primary CNS lymphoma in HIV/AIDS patients is crucial for improving patient care.
Purpose:
- To retrospectively review the clinical, neuroimaging, diagnostic, and histological features of primary CNS lymphoma in HIV/AIDS patients.
- To determine the association of Epstein-Barr virus (EBV) with primary CNS lymphoma in this population.
- To evaluate the prognosis and survival rates of patients diagnosed with primary CNS lymphoma.
Summary:
- The study reviewed 18 HIV/AIDS patients with primary CNS lymphoma, finding an overall incidence of 2.6%.
- Common clinical findings included focal neurological deficits and seizures, with a mean CD4 T cell count of 44 cells/microl.
- Large lesions (> 2.5 cm) with mass effect were typical. Epstein-Barr virus (EBV) DNA was detected in brain smears and cerebrospinal fluid (CSF) in most cases.
- Median survival after diagnosis was 75 days, indicating a poor prognosis.
Impact:
- This study reinforces the link between EBV and primary CNS lymphoma in AIDS patients.
- The findings highlight the aggressive nature and poor survival associated with primary CNS lymphoma in this vulnerable population.
- Results underscore the need for early diagnosis and targeted therapeutic strategies for primary CNS lymphoma in HIV/AIDS patients.
