Summary
Regional lymph node metastasis (RLNM) in soft tissue sarcoma is rare. Isolated RLNM offers improved survival, while synchronous RLNM with distant metastasis indicates a poorer prognosis.
Area of Science:
- Oncology
- Surgical Pathology
- Cancer Prognostics
Background:
- Regional lymph node metastasis (RLNM) is an uncommon event in soft tissue sarcoma (STS).
- Understanding the characteristics and outcomes of STS patients with RLNM is crucial for treatment planning.
Purpose of the Study:
- To define the clinicopathological features of soft tissue sarcoma with regional lymph node metastasis.
- To evaluate the prognostic impact of RLNM on patient survival.
Main Methods:
- Retrospective analysis of patients diagnosed with soft tissue sarcoma and regional lymph node metastasis.
- Data extracted from a prospective institutional database (Royal Marsden Hospital Sarcoma Unit) from January 1990 onwards.
- Statistical analysis of survival rates based on metastasis presentation (isolated vs. synchronous with distant metastasis) and timing (synchronous vs. metachronous).
Main Results:
- Out of 2127 patients, 73 (3.4%) had RLNM. The most common histologies were rhabdomyosarcoma, epithelioid sarcoma, and angiosarcoma.
- RLNM occurred as the first site of spread in 78.1% of cases. 21.9% presented with synchronous regional and distant metastasis.
- One-year survival was significantly higher for isolated RLNM (77.49%) compared to RLNM with distant metastasis (36.27%) (P=.005). Metachronous RLNM showed better survival (94.44%) than synchronous RLNM (67.54%) (P=.05).
Conclusions:
- Regional lymph node metastasis is an infrequent occurrence in soft tissue sarcoma.
- Isolated RLNM is associated with a better prognosis than combined regional and distant metastasis at diagnosis.
- Synchronous RLNM portends a poorer outcome compared to metachronous RLNM when distant metastasis is absent.
