Summary

Neonatal lupus erythematosus (NLE) is caused by maternal autoantibodies crossing the placenta, leading to fetal complications like skin issues and congenital heart block (CHB). While skin and other symptoms are temporary, CHB is permanent and requires lifelong management.

Area of Science:

  • Immunology
  • Pediatrics
  • Dermatology

Background:

  • Neonatal lupus erythematosus (NLE) is a rare condition resulting from the passive transfer of maternal autoantibodies, primarily anti-Ro/SSA and anti-SSB/La.
  • These autoantibodies cross the placenta, potentially causing fetal tissue damage and characteristic NLE manifestations.
  • NLE presents with diverse clinical features, including transient cutaneous lesions, hematologic abnormalities, hepatic dysfunction, and a significant risk of permanent congenital heart block (CHB).

Purpose of the Study:

  • To review the pathogenesis of neonatal lupus erythematosus (NLE).
  • To summarize the clinical manifestations and diagnostic considerations of NLE.
  • To highlight the role of maternal autoantibodies in NLE and congenital heart block (CHB).

Main Methods:

  • Review of existing literature on neonatal lupus erythematosus (NLE).
  • Analysis of pathogenic mechanisms involving maternal autoantibodies (anti-Ro/SSA, anti-SSB/La).
  • Correlation of clinical findings with serological markers and disease outcomes.

Main Results:

  • NLE affects multiple organs, with cutaneous manifestations resembling subacute cutaneous lupus erythematosus (SCLE) in approximately 50% of cases.
  • Congenital heart block (CHB) occurs in about 50% of NLE patients, representing 80% of all CHB cases, and is a permanent complication.
  • Cutaneous, hematologic, and hepatic issues typically resolve by six months of age, whereas CHB often necessitates pacemaker implantation.

Conclusions:

  • Neonatal lupus erythematosus is primarily mediated by passively transferred maternal autoantibodies.
  • Congenital heart block is the most severe and permanent complication of NLE, underscoring the importance of early detection and management.
  • Understanding the role of maternal autoantibodies is crucial for predicting and managing NLE and its associated complications in newborns.
