Migrainous vertigo, a common cause of episodic dizziness, presents heterogeneously. We propose diagnostic criteria for migrainous vertigo in migraine patients, aiding recognition and management.

Area of Science:

  • Neurology
  • Otolaryngology
  • Neuroscience

Context:

  • Migrainous vertigo is a frequent cause of episodic vertigo.
  • Its clinical presentation is highly variable, with vertigo episodes lasting from seconds to days.
  • A clear temporal link to headache or other migraine features is often inconsistent.

Purpose:

  • To propose diagnostic criteria for definite migrainous vertigo.
  • To facilitate the recognition of migrainous vertigo by clinicians.
  • To address the lack of evidence-based therapeutic guidelines.

Summary:

  • Migrainous vertigo is characterized by recurrent vestibular vertigo episodes in patients with a history of migraine.
  • The proposed diagnostic criteria require at least two vertigo attacks accompanied by migrainous symptoms, excluding other causes.

Related Experiment Videos

  • Current classification systems, including the International Headache Society (IHS), do not yet include migrainous vertigo.
  • Impact:

    • Improved diagnostic accuracy for migrainous vertigo.
    • Potential for developing evidence-based treatment guidelines.
    • Enhanced understanding of the pathophysiology of vestibular migraine.
