Background:

Primary cardiac tumors are rare.
Cardiac rupture is an exceptionally rare complication of cardiac tumors.

Observation:

A 58-year-old patient presented with dyspnea, weight loss, anorexia, and asthenia.
Chest X-ray showed global cardiac enlargement, predominantly of the right atrium.
Transthoracic and transesophageal echocardiography, with contrast, revealed a right atrial pseudoaneurysm and the ruptured atrial wall.

Findings:

Magnetic resonance imaging (MRI) was crucial for detailed anatomic evaluation.
Histological confirmation established the diagnosis of angiosarcoma.
This case represents an uncommon presentation of cardiac angiosarcoma as a right atrial rupture.
