This study found no strong link between nevirapine (NVP) levels and liver damage in HIV patients. However, higher unbound NVP concentrations were seen in those with elevated liver enzymes or hepatitis B/C coinfection.

Purpose of the Study:

  • To investigate the association between total and unbound plasma nevirapine concentrations and liver enzymes alanine aminotransferase (ALT) and gamma-glutamyl transferase (gammaGT) in HIV-infected individuals.
  • To explore the impact of hepatitis B or C virus (HBV/HCV) coinfection on NVP concentrations and liver function.

Main Methods:

  • Prospective study of 85 HIV-infected individuals receiving NVP and nucleoside reverse transcriptase inhibitors (NRTIs).
  • Measurement of total and unbound NVP trough concentrations (C(trough)) using HPLC-MS/MS.

    Conclusions:

    • No strong overall association was found between nevirapine concentrations and hepatotoxicity.
    • While NVP was well-tolerated in coinfected patients, higher plasma concentrations were noted.
    • Further research may be needed to fully elucidate NVP pharmacokinetics in specific patient populations.
