Summary

Triple valve surgery for rheumatic heart disease is complex but yields encouraging long-term survival rates. Replacing the tricuspid valve with a mechanical prosthesis is preferred to avoid complications associated with bioprostheses.

Background:

  • Rheumatic valvular disease necessitates significant cardiac operations in developing nations.
  • Triple valve surgery (aortic, mitral, tricuspid) presents challenges due to prolonged cardiopulmonary bypass and myocardial decompensation.

Purpose of the Study:

  • To assess the outcomes of triple valve surgery.
  • To compare outcomes between triple valve replacement and combined aortic/mitral replacement with tricuspid annuloplasty.

Main Methods:

  • A retrospective analysis of 34 patients undergoing triple valve surgery between 1977 and 2002.
  • 11 patients had triple valve replacement; 23 had tricuspid valve annuloplasty with aortic and mitral valve replacements.

  • Hospital mortality was 11.8% (4 deaths).
  • Actuarial survival rates at 5, 10, and 15 years were 85%, 72%, and 48%, respectively.
  • Actuarial freedom from reoperation was 86.3% (5 years), 71.9% (10 years), and 51.2% (15 years). Major cerebral complications occurred in 10 patients.

Conclusions:

  • Preference for tricuspid valve replacement with a bileaflet mechanical prosthesis over repair when indicated.
