Summary

Laparoscopic surgery for renal adenocarcinoma provides oncological outcomes equivalent to open surgery for non-metastatic tumors. Both radical and partial laparoscopic nephrectomies demonstrate comparable recurrence-free survival rates to their open counterparts.

Area of Science:

  • Urology
  • Surgical Oncology

Background:

  • Renal adenocarcinoma treatment traditionally involves open surgery.
  • Minimally invasive laparoscopic techniques are increasingly adopted for various surgical procedures.

Purpose of the Study:

  • To compare the oncological outcomes and perioperative results of laparoscopic versus open surgery for renal adenocarcinoma.
  • To evaluate the efficacy of laparoscopic partial nephrectomy against open partial nephrectomy.

Main Methods:

  • Retrospective analysis of 327 patients with renal adenocarcinoma.
  • Patients underwent either laparoscopic or open radical/partial nephrectomy.
  • Comparison of tumor size, blood loss, hospital stay, complications, and oncological outcomes.

Main Results:

Related Experiment Videos

  • Laparoscopic surgery was associated with smaller median tumor size (4.1 cm vs 9.6 cm), less blood loss (254 ml vs 460 ml), and shorter hospital stay (4.5 days vs 10 days) compared to open surgery.
  • Complication rates were similar between laparoscopic and open surgery groups (21.5% vs 27.5%).
  • Stage-for-stage, oncological results, including 5-year recurrence-free survival rates (e.g., 97% open radical vs 96.8% laparoscopic radical for PT1), were identical.

Conclusions:

  • Laparoscopic surgery for renal adenocarcinoma achieves identical oncological results to open surgery for non-metastatic tumors <7 cm.
  • Laparoscopic partial nephrectomy is as effective as open partial nephrectomy.
  • Laparoscopic nephrectomy offers significant advantages in reduced blood loss and shorter hospital stay.
