Diagnosing hypothyroidism in the elderly is challenging. While many older adults show abnormal thyroid lab tests, the actual prevalence of hypothyroidism is low, with TSH levels over 3 IU/ml being a key indicator.

Area of Science:

  • Gerontology
  • Endocrinology
  • Clinical Medicine

Background:

  • Hypothyroidism diagnosis in elderly populations presents unique challenges.
  • Altered thyroid function tests are common in older adults, but clinical hypothyroidism is less frequent.

Purpose of the Study:

  • To evaluate the prevalence of altered thyroid parameters and clinical hypothyroidism in subjects over 65 years of age.
  • To identify reliable indicators for diagnosing hypothyroidism in the elderly.

Main Methods:

  • Analysis of total triiodothyronine (T3), total thyroxine (T4), and thyroid-stimulating hormone (TSH) in 3015 subjects over 65.
  • Recording clinical signs of hypothyroidism in a subsample of 300 elderly individuals.

Main Results:

Related Experiment Videos

  • A high prevalence of abnormal thyroid lab results (17.88%) was observed.
  • The actual prevalence of clinical and laboratory hypothyroidism was significantly lower at 1.00%, with a 2:1 female-to-male ratio.
  • The most common laboratory finding was 'altered pituitary' status, and neurological symptoms were the most frequent clinical sign.

Conclusions:

  • Diagnosing hypothyroidism in the elderly is complex due to discrepancies between lab results and clinical presentation.
  • TSH values exceeding 3 IU/ml, as determined by the IRMA method, appear to be the most indicative laboratory marker for hypothyroidism in this age group.
