Summary

Diffuse panbronchiolitis (DPB) is treatable with macrolides, but underdiagnosis is common outside Japan. This study highlights DPB clinical features in Chinese patients to improve early recognition and diagnosis of this rare lung disease.

Area of Science:

  • Pulmonology
  • Respiratory Medicine
  • Thoracic Surgery

Background:

  • Diffuse panbronchiolitis (DPB) is a rare inflammatory lung disease predominantly reported in Japan.
  • Effective macrolide therapy necessitates increased global awareness and familiarity with DPB's clinical presentation.
  • Urgent need to investigate DPB in non-Japanese populations due to its treatable nature.

Purpose of the Study:

  • To investigate the clinical features of diffuse panbronchiolitis (DPB) in a Chinese population.
  • To propose diagnostic procedures to enhance clinician awareness and facilitate earlier diagnosis of DPB.
  • To address the underdiagnosis of this treatable respiratory condition.

Main Methods:

  • Retrospective analysis of 9 histopathologically confirmed diffuse panbronchiolitis (DPB) cases.

Related Experiment Videos

  • Confirmation of DPB via open lung biopsy or video-assisted thoracic surgical biopsy.
  • Literature review and discussion of diagnostic procedures for DPB.
  • Main Results:

    • Key clinical features included persistent cough, sputum, exertional dyspnea (89%), and chronic sinusitis history (78%).
    • Chest CT revealed centrilobular micronodules (100%), and physical exams showed coarse crackles (78%).
    • Diagnostic criteria identified DPB definitively in 44%, suggested in 33%, but 78% were initially missed or misdiagnosed.

    Conclusions:

    • Diffuse panbronchiolitis (DPB) can be diagnosed using clinical criteria, though features vary by ethnicity and disease stage.
    • Underdiagnosis of DPB is prevalent due to unfamiliarity with its clinical and diagnostic characteristics.
    • Clinicopathological correlation and exclusion of other diseases are crucial for difficult DPB diagnoses; surgical biopsy may be needed.
