Soluble P-selectin, a marker of platelet activity, shows significant diurnal variation in patients with coronary artery disease (CAD). Levels are higher in the evening, suggesting shed forms of membrane-bound P-selectin.

Area of Science:

  • Cardiology
  • Biochemistry
  • Vascular Biology

Background:

  • Acute coronary syndromes (ACS) frequently occur in the morning.
  • Diurnal variations in fibrinolysis are known, but less is understood about soluble adhesion molecules in coronary artery disease (CAD).
  • Soluble adhesion molecules may indicate endothelial or platelet activity.

Purpose of the Study:

  • To investigate the diurnal variation of soluble adhesion molecules in patients with CAD.
  • To assess plasma concentrations of soluble P-selectin, E-selectin, and intercellular adhesion molecule-1 at different times of day.

Main Methods:

  • 80 patients with a history of myocardial infarction or chest pain were studied.
  • Blood samples were collected at 7:00 a.m. and 7:00 p.m. from 70 CAD patients and 10 controls.
  • Plasma levels of soluble P-selectin (sP-selectin), E-selectin (sE-selectin), and intercellular adhesion molecule-1 (sICAM-1) were measured.

Main Results:

  • No diurnal variation was observed for sE-selectin and sICAM-1 in either group.
  • Soluble P-selectin (sP-selectin) levels were significantly higher in the evening for both the CAD group and the control group.
  • CAD group evening sP-selectin: 172.2 +/- 68.8 ng/ml; Control group evening sP-selectin: 187.5 +/- 96.3 ng/ml.

Conclusions:

  • Diurnal variation of sP-selectin was demonstrated in patients with CAD.
  • Elevated evening sP-selectin levels likely represent shed forms of membrane-bound P-selectin.
  • This finding contributes to understanding the diurnal patterns of platelet activity in CAD.
