Delayed immunologic thrombocytopenia induced by abciximab.

Summary

Delayed thrombocytopenia can occur after abciximab (anti-GPIIb-IIIa drug) infusion due to antibodies against abciximab-coated platelets. This immune response may also lead to platelet activation and aggregation, suggesting a prothrombotic risk.

Characteristics and Outcomes of Operated Versus Nonoperated Patients With Infective Endocarditis and Cerebral Complications: the COOPER Study.

The Canadian journal of cardiology·2026

Area of Science:

  • Immunology
  • Hematology
  • Cardiology

Background:

  • Abciximab is an anti-platelet drug used to prevent clots during cardiac procedures.
  • Thrombocytopenia (low platelet count) is a known complication, but its delayed onset and immunologic basis require further investigation.

Purpose of the Study:

  • To investigate the immunologic origin of delayed thrombocytopenia after abciximab administration.
  • To characterize the antibodies involved and their potential prothrombotic effects.

Main Methods:

  • Monoclonal antibody immobilization of platelet antigens assay (MAIPA) using SZ22.
  • Flow cytometry to detect IgG antibodies against abciximab-sensitized platelets.
  • Electron microscopy and immunogold labeling to assess platelet activation and aggregation.

Related Experiment Videos

Main Results:

  • Antibodies against abciximab-coated platelets were found in patients with delayed thrombocytopenia (7-12 days post-infusion).
  • These antibodies were also present in a patient with acute thrombocytopenia.
  • Antibody-containing plasma induced abciximab-dependent platelet activation and aggregation, with abciximab associated with platelet aggregates.

Conclusions:

  • Abciximab-induced thrombocytopenia can have a delayed onset mediated by specific IgG antibodies.
  • The presence of these antibodies may indicate a prothrombotic risk due to platelet activation and aggregation.
