    Background:

    • Dan Shen, a traditional Chinese herbal medicine, is widely used in China for acute ischemic stroke.
    • Experimental data suggests Dan Shen improves cerebral microcirculation.
    • Evidence from randomized controlled trials (RCTs) needs systematic assessment.

    Purpose of the Study:

    • To review RCTs of Dan Shen agents for acute ischemic stroke.
    • To determine if Dan Shen improves functional outcomes and reduces harm.
    • To assess effects on neurological impairment and quality of life.

    Main Methods:

    • Searched multiple databases including Cochrane, MEDLINE, EMBASE, CINAHL, AMED, and CBM-disc up to September 2003.
    • Included randomized or quasi-randomized controlled trials comparing Dan Shen agents to placebo or no placebo.
    • Two independent reviewers selected trials, assessed quality, and extracted data.

    Main Results:

    • Three trials with 304 patients were included; data on deaths and long-term dependency were not reported.
    • One trial reported adverse events.
    • Compound Dan Shen agents significantly increased the number of patients with improved neurological deficits (OR 2.72, 95% CI 1.10 to 6.72).
    • No deaths were reported within the follow-up period (21-28 days).
    • Quality of life was not assessed.

    Conclusions:

    • Insufficient patient numbers and outcome events preclude reliable conclusions.
    • Included studies exhibited poor methodological quality.
    • High-quality RCTs are required to validate the efficacy of Dan Shen for acute ischemic stroke.
