Background:

Intercostal hemangiomas are rare benign vascular tumors.
Chest pain and shortness of breath can be presenting symptoms.

Observation:

A 27-year-old male presented with chest pain and dyspnea.
Computed tomography revealed a 5.5 x 3.5 cm soft tissue mass in the right 7th intercostal space.
Dynamic MRI demonstrated characteristic rapid early enhancement and sustained delayed enhancement, suggestive of hemangioma.

Findings:

Complete surgical resection of the intercostal hemangioma was achieved.
The resection involved the tumor, 7th and 8th ribs, and intercostal muscles.
Histopathological examination confirmed an intramuscular hemangioma of the large-vessel type.
