Area of Science:

  • Pediatric Urology
  • Hematology
  • Surgical Innovation

Background:

  • Priapism is a prolonged erection, a rare condition in childhood.
  • Sickle cell disease is the most common underlying condition in pediatric priapism cases.
  • Early presentation often allows for successful nonoperative management.

Observation:

  • A 14-year-old male with sickle cell disease presented with priapism lasting 72 hours.
  • The patient's condition represented a prolonged and severe priapism episode.

Findings:

  • The patient underwent surgical treatment with bilateral saphenocorporal shunts.
  • This surgical approach resulted in a successful resolution of the priapism.
  • The intervention was effective despite the 72-hour duration of the condition.

Implications:

  • Bilateral saphenocorporal shunts are a viable and effective treatment option for prolonged priapism in pediatric patients with sickle cell disease.
  • This case expands treatment options for refractory priapism in this vulnerable population.
  • Early surgical consideration may be warranted in complex pediatric priapism cases unresponsive to initial management.
