Summary

Intense emotional stress can cause severe heart muscle dysfunction, leading to cardiogenic shock. This condition, known as transient left ventricular apical ballooning, often resolves completely with supportive care.

Background:

  • Takotsubo cardiomyopathy, or transient left ventricular apical ballooning, is a stress-induced cardiac dysfunction.
  • This syndrome is characterized by acute left ventricular dysfunction following intense emotional or physical stress.

Observation:

  • A patient presented with severe ventricular dysfunction and cardiogenic shock after significant emotional stress.
  • Clinical presentation was consistent with transient left ventricular apical ballooning, with normal coronary arteries and no identified infectious agent.

Findings:

  • Complete recovery of left ventricular systolic function was observed.
  • [I123]metaiodobenzyl guanidine cardiac scintigraphy revealed a significant reduction in cardiac sympathetic nerve activity.
