Splenic hamartomas, rare spleen tumors, can present with unusual bizarre stromal cells. Recognizing this variant is crucial to avoid misdiagnosing it as malignancy.

Background:

  • Splenic hamartoma is a rare benign tumor of the spleen.
  • It is characterized by disorganized red pulp tissue without interspersed white pulp.

Observation:

  • This report details three cases of splenic hamartoma with previously unreported scattered bizarre stromal cells.
  • These lesions were solitary, circumscribed, and unencapsulated, featuring vascular channels and a loose stroma.
  • The bizarre stromal cells exhibited atypical nuclear features but lacked mitotic activity.

Findings:

  • The bizarre stromal cells were negative for a wide range of markers including lymphoid, dendritic cell, histiocytic, myeloid, endothelial, epithelial, and melanocytic markers.
  • Rare bizarre cells showed focal desmin positivity in one case.
