Summary

Diagnosing metastatic epithelioid angiosarcoma in the lungs is challenging. This case highlights immunohistochemistry and electron microscopy for identifying this rare vascular tumor, even when the primary site is unknown.

  • Neoplastic cells showed positivity for vimentin and CD31, but negative for epithelial, melanocytic, lymphoid markers, CD34, and factor VIIIR:Ag.

Findings:

  • Electron microscopy revealed Weibel-Palade bodies and pinocytosis, confirming epithelioid angiosarcoma.
  • Post-diagnosis Doppler studies identified potential primary tumor in the left calf musculature.
