Amyloid Fibrils

Amyloid fibrils are aggregates of misfolded proteins.  Under most circumstances, misfolded proteins are either refolded by chaperone proteins or degraded by the proteasome. However, in the case of a mutation or a disease, these proteins can accumulate to form large clusters and often further assemble to form elongated fibers, called fibrils.

Summary

Amyloidosis is rarely diagnosed in FDG-avid lymph nodes. This case highlights the importance of considering amyloidosis in FDG-avid lymphadenopathy, especially when malignancy is suspected.

Background:

  • Amyloidosis is a rare condition characterized by the abnormal deposition of proteins in various organs.
  • FDG-avid lymphadenopathy is typically associated with malignancy, infection, or inflammatory conditions.

Observation:

  • A 64-year-old woman presented with left axillary pain.
  • FDG PET/CT revealed multifocal lymphadenopathy, primarily axillary, and a mesenteric mass.

Findings:

  • Histological examination of a surgical lymph node biopsy confirmed amyloidosis.
  • This diagnosis was made despite the initial suspicion of malignancy due to FDG avidity.

Implications:

  • Amyloidosis should be included in the differential diagnosis of FDG-avid lymphadenopathy.
  • This finding challenges the conventional interpretation of FDG PET/CT findings in lymph nodes.
  • Further research may elucidate the mechanisms behind FDG avidity in amyloidosis.
