Summary

Frontotemporal dementia (FTD) can present with severe amnesia, challenging typical diagnostic criteria. This study found that memory loss is a common initial symptom in FTD, suggesting a need for revised diagnostic guidelines.

Area of Science:

  • Neurology
  • Neuroscience
  • Pathology

Background:

  • Frontotemporal dementia (FTD) diagnosis typically excludes early, severe memory impairment.
  • Clinical observations suggest FTD can manifest with amnesia, contradicting established diagnostic exclusion criteria.

Observation:

  • This study reviewed neuropathologically confirmed FTD cases to identify patients presenting with dominant memory complaints.
  • Eight out of 71 FTD patients exhibited severe amnesia at presentation, with memory loss as their sole or primary symptom.

Findings:

  • Patients initially misdiagnosed with Alzheimer's disease or enrolled in anticholinesterase trials often developed behavioral features later.
  • Neuropathological examination confirmed FTD in all cases, highlighting the diagnostic challenge posed by initial amnesia.

Related Experiment Videos

Implications:

  • The findings suggest that severe amnesia at presentation is more common in FTD than previously recognized.
  • Current clinical consensus criteria for FTD diagnosis may require revision to incorporate amnesic presentations.
  • The etiological basis for memory impairment in FTD may be diverse, varying across patient subgroups.
