Summary

Multiple spinal dural arteriovenous fistulas (DAVFs) are rare, with this case showing recurrence and a new fistula after initial treatment. Patients with recurring symptoms post-DAVF treatment require further angiography to detect additional fistulas.

Background:

  • Spinal dural arteriovenous fistulas (DAVFs) are uncommon vascular malformations.
  • Multiple spinal DAVFs are exceptionally rare, with only two prior reported cases.

Observation:

  • A 71-year-old male presented with bilateral leg weakness and hypesthesia.
  • Initial diagnosis was a thoracic spinal DAVF, treated surgically.
  • Symptoms recurred 14 months post-treatment, revealing a new lumbar spinal DAVF, also treated surgically.

Findings:

  • This case highlights the potential for multiple spinal DAVFs.
  • In previously reported cases and this one, secondary fistulas were diagnosed after initial treatment for a primary spinal DAVF.

Implications:
