Summary

Hyperbaric Oxygen Therapy (HBO2) did not improve neurologic deficits or cerebral blood flow in chronic traumatic brain injuries (TBI) patients in this pilot study. Further research is needed to explore potential TBI treatments.

Background:

  • Chronic traumatic brain injuries (TBI) can lead to persistent neurologic deficits and altered cerebral blood flow.
  • Hyperbaric Oxygen Therapy (HBO2) is a potential treatment modality, but its efficacy in TBI requires further investigation.

Purpose of the Study:

  • To evaluate the effectiveness of HBO2 in improving neurologic deficits and regional cerebral blood flow (rCBF) in individuals with chronic TBI.
  • To assess changes in neurological function, neuropsychometric performance, and cerebral perfusion following HBO2 treatment.

Main Methods:

  • A nonrandomized control pilot trial was conducted with five TBI subjects receiving HBO2.
  • Control groups included five matched head-injured controls (HIC) and five healthy normal controls.
  • SPECT brain scans were used to analyze rCBF, alongside neurologic, neuropsychometric, and exercise testing, and MRI/CT scans.

Main Results:

  • No significant objective improvements were observed in neurologic, neuropsychometric, or exercise testing outcomes.
  • Regional cerebral blood flow (rCBF) analysis using Statistical Parametric Mapping on SPECT scans showed no significant changes.
  • No significant changes were detected in MRI or CT scans following HBO2 treatment.

Conclusions:

  • This small pilot study suggests that HBO2 did not significantly improve clinical outcomes or rCBF in patients with chronic TBI.
  • The findings indicate a need for more extensive research to determine the role of HBO2 in TBI management.
