    Summary

    The Collaborative AtoRvastatin Diabetes Study (CARDS) found that atorvastatin significantly reduced cardiovascular events in type 2 diabetes patients. This statin therapy proved beneficial for primary prevention, even with average cholesterol levels.

    Background:

    • Type 2 diabetes is a major risk factor for cardiovascular disease.
    • Primary prevention strategies are crucial for managing cardiovascular risk in diabetic patients.
    • Statins are widely used for cholesterol management, but their role in primary prevention for certain diabetic populations requires specific investigation.

    Purpose of the Study:

    • To evaluate the efficacy of atorvastatin in primary prevention of cardiovascular events in patients with type 2 diabetes.
    • To assess the impact of atorvastatin on cardiovascular outcomes in individuals without pre-existing vascular disease and with average or below-average cholesterol levels.

    Main Methods:

    • Prospective, double-blind, randomized, placebo-controlled trial (Collaborative AtoRvastatin Diabetes Study - CARDS).
    • 2383 type 2 diabetic subjects aged 40-75 years randomized to 10-mg atorvastatin daily or placebo.

    • The observed benefits may be underestimated due to protocol allowances for statin use in the placebo group and discontinuations in the active group.
