Abstract:
Fracture of the distal end of the forearm secondary to a fall on the outstretched hand is common in elderly patients with osteoporosis. The distal fragments of the fracture are usually displaced dorsally. Although fragment reunion may be successful in such cases, the displacement often recurs after reduction. The main objective of the treatment, therefore, is to ensure minimal displacement. Conservative therapy includes manipulative reduction under anesthesia and subsequent external immobilization for 4 to 6 weeks. During the period of immobilization, vigilance should be maintained against the development of swelling and contracture. In this report, we present the procedures and results of conservative therapy using a cast with the wrist in dorsiflexion. We also comment on the surgical treatment of this fracture which has drawn much attention in recent years.
