Summary

Gallbladder cancer incidence is higher in acute cholecystitis patients in Hong Kong. Early diagnosis during acute cholecystitis may allow for long-term survival, even with laparoscopic cholecystectomy (LC).

Area of Science:

  • Gastroenterology
  • Surgical Oncology
  • Hepatobiliary Surgery

Background:

  • The role of laparoscopic cholecystectomy (LC) in acute cholecystitis is debated.
  • Gallbladder cancer (GC) diagnosis often occurs incidentally during cholecystectomy for gallstones.
  • Acute cholecystitis may present a unique window for GC detection.

Purpose of the Study:

  • To determine the incidence and clinicopathological characteristics of gallbladder cancer in patients presenting with acute cholecystitis.
  • To evaluate the outcomes of patients with gallbladder cancer diagnosed during acute cholecystitis, including those treated with LC.
  • To compare outcomes between open and laparoscopic cholecystectomy for acute cholecystitis with incidental gallbladder cancer.

Main Methods:

  • Retrospective analysis of patients with gallbladder cancer presenting with acute cholecystitis.

Related Experiment Videos

  • Data collected from public hospitals in Hong Kong between 1998 and 2002.
  • Comparison of outcomes (complications, mortality, survival) between open and LC groups.
  • Main Results:

    • 63 of 2,700 (2.3%) patients with acute cholecystitis had gallbladder cancer.
    • Adenocarcinoma was the most common type (90.5%). Median survival was 5 months; 5-year survival was 20.8%.
    • LC was attempted in 11 patients (6 successful) with no significant difference in outcomes compared to open surgery.

    Conclusions:

    • The incidence of gallbladder cancer presenting with acute cholecystitis is higher in Hong Kong's ethnic Chinese population compared to elective cases.
    • Early diagnosis of gallbladder cancer during acute cholecystitis is possible.
    • Long-term survival is achievable, suggesting potential for early-stage detection and treatment.
