Background:

  • Primary hyperparathyroidism (PHPT) is commonly caused by parathyroid adenoma.
  • Most PHPT cases are asymptomatic, but aggressive forms exist.

Observation:

  • A 41-year-old man presented with recurrent ureteral stones and severe hypertension.
  • Laboratory results revealed hypercalcemia, hypophosphatemia, and elevated parathyroid hormone levels.

Findings:

  • Ultrasound successfully localized a parathyroid adenoma.
  • Surgical removal of the adenoma confirmed the diagnosis and resolved symptoms.

Implications:

  • Recurrent ureteral stones can be an indicator of severe PHPT.
  • Early investigation of kidney stones is crucial to prevent PHPT complications.
