  • Chest X-ray revealed a nasogastric tube within the left hemithorax.
  • Laparotomy confirmed herniation of spleen, stomach, and intestines into the left pleural space through a diaphragmatic defect.

Implications:

  • Accurate diagnosis of diaphragmatic defects is vital to avoid surgical complications.
  • Mismanagement, including inappropriate tube thoracostomy, can worsen outcomes in diaphragmatic herniation.
