Summary
Gastrointestinal stromal tumors (GISTs) are rare small intestine tumors. This case highlights a duodenal GIST presenting with obstructive symptoms and anemia, emphasizing its consideration in differential diagnoses.
Area of Science:
- Gastroenterology
- Surgical Oncology
- Pathology
Background:
- Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the digestive tract, though duodenal GISTs are relatively rare.
- Small intestine tumors, including GISTs, can present with varied gastrointestinal symptoms and anemia.
Observation:
- A 78-year-old female presented with a three-month history of postprandial abdominal pain, nausea, vomiting, and weight loss.
- Physical examination revealed epigastric tenderness; laboratory tests showed anemia and hypoalbuminemia. Abdominal CT identified a large duodenal mass.
- Endoscopic biopsy confirmed a polypoid, vegetative mass in the second portion of the duodenum, diagnosed as a GIST.
Findings:
- The patient underwent a pancreaticoduodenectomy for the duodenal GIST.
- Postoperative complications included biliary leakage from the choledochojejunostomy, requiring revision surgery.
- Histopathological examination of the resected specimen confirmed a duodenal stromal tumor.
Implications:
- Duodenal GISTs, while uncommon, should be included in the differential diagnosis for patients presenting with upper gastrointestinal obstruction and anemia.
- Prompt diagnosis and surgical management are crucial for managing GISTs.
- This case underscores the importance of considering rare small bowel tumors in the evaluation of persistent gastrointestinal symptoms.
